Anticoagulation Clinic Follow Up at VAMC

The following describes the process for obtaining anticoagulation clinic follow up at VAMC Albuquerque for patients enrolled within the VA who are hospitalized at UNMH.


-For an enrolled veteran hospitalized at UNMH who requires anticoagulation follow up at the VAMC-Albuquerque, UNM provider (or designee) will contact ‚Äčthe VA anticoagulation clinic directly at 505-265-1711 x 4429 to set up a follow up appointment prior to discharge.  UNM PCAP will fax any related anticoagulation documentation to VA anticoagulation clinic. Follow up appointment information will be communicated to the veteran in discharge paperwork. 


Please note, patients must be enrolled in the VA system to obtain anticoagulation follow up.  An assigned VA PCP, however, is not required to set up an the appointment.
